With this study, using methods applicable in routine clinical and laboratory settings, we have demonstrated detection and quantification SARS-CoV-2 genomes in the exhaled breath of infected patients, one third of whom were asymptomatic at time of sampling. We found that patients exhaled viral RNA ranging over five orders in magnitude and that this appeared to be highest in the first few days of symptoms and when active respiratory symptoms were present at time of sampling. In contrast to concomitant NPS viral loads, we also found that higher FMS viral loads may have an association with more severe disease.
FMS was able to detect SARS-CoV-2 in the exhaled breath of 38% of individuals that are known to have COVID-19. This contrasts with the 26.9% positive rate in infected individuals, reported by Ma and colleagues who sampled for five minutes using a breath condensate sampling device.5 We note the use of a bespoke sampling device in the Ma study and the application of processing methods largely outside those generally available in routine settings. Our results are similar to the preliminary report of Sriraman and colleagues, who found 40% positivity rate amongst infected individuals by FMS using a gelatine sampling matrix.17
Previous studies have shown that NPS signals can be persistently high for several weeks despite index cases that are no longer infectious.18, 19, 20 Kim and colleagues found that in hospitalised patients with COVID-19, the median time from symptom onset to viral clearance was 7 days.21 In our study, no patient was FMS positive for more than 10 days after symptom onset. FMS viral load was highest in early disease (including in those who were nosocomial infected) and more virus was exhaled in those with active respiratory symptoms at time of sampling than those who were asymptomatic. A higher viral load in those who coughed at time of sampling may reflect evidence that FMS captures a combination of aerosols and larger droplets, likely sourced from the oropharynx and lower respiratory tract and depending on contemporaneous respiratory efforts, vocalisation and respiratory symptom severity.26 We also did not record whether participants spoke during the period of sampling, which may have affected viral load on FMS.
The quantities of exhaled virus detected were not always predicted by quantitation by NPS. Linear regression analysis indicated that the NPS-detected viral RNA only accounts for 15% of the variation detected by FMS (r2 = 0.15). In particular, the detection of five individuals whose FMS viral loads greatly exceeded those predicted by the general relationship between FMS and NPS. FMS viral loads were low when NPS was negative; NPS viral load on the other hand varied significantly between samples when concomitant FMS was negative.
Altogether, these findings suggest that FMS viral load patterns emitted from individuals may be distinct from NPS. Future work will explore the relationship between patterns of FMS output of SARS-CoV-2 and transmission to determine the value of this approach in determining infectivity of individuals, compared to other tests proposed for similar purposes, such as the lateral flow assays.22
In multivariable analyses, higher FMS viral loads, but not those detected by NPS, were significantly related to both ISARIC mortality and deterioration scores. Previous studies also show little differences in NPS viral load between those with mild, moderate and severe disease.23, 24, 25 NPS-detected viral loads in patients with COVID-19 rapidly decrease in the upper airways in those with increasing disease severity as the viral load shifts to the lower respiratory tract.23, 24, 25 We have previously detected surfactant protein A in FMS samples, indicating the likelihood that FMS captures at least some of its content from the lower respiratory tract.26 This feature provides a potential explanation for why higher FMS viral loads detected here correlated with higher ISARIC scores and therefore predicted higher disease severity in contrast to NPS which does not. Whilst larger, formally powered studies are needed, FMS may have a role in early identification of individuals at risk of deterioration, hospitalisation and death.
Face-masks are widely recommended for limiting transmission from source patients, and consequently, now accepted by society. Our method is inexpensive, simple and easy to replicate. The potential detection of infectious individuals, or those that may go on to develop severe disease could help with infection control and initiate life-saving treatment earliar in those who present to hospital. For example, FMS could be provided to patients in the waiting rooms of emergency departments, clinics or offices as additional infection control measures, and the PVA strips could be removed for routine processing in laboratories.
Our study was limited by size. As a pilot study using a novel device, we were unable to undertake formal power calculations. Yield from FMS may have been affected by different behaviours of our subjects during sampling. Although we recorded presence or absence of cough, other potentially relevant activities such as vocalisation were not assessed. Our sampling protocol has been developed to involve minimal constraint of subjects to maximise acceptability and reflect individuals’ unsolicited natural emissions with potential relevance to transmission. We acknowledge that diagnostic yield might be maximised by requiring specific respiratory manoeuvres. We assessed disease severity using ISARIC scoring systems rather than clinical outcomes of intubation or death, because our cohort had a small number of deaths. Many of our patients were elderly with multiple comorbidities – thus our results may not be generalisable to younger, fitter patients in the community. Ethnic minority groups, especially those from Asian ethnic groups are generally more likely to have worse outcomes from COVID-19 compared to White groups27
28; the discordant findings here likely reflect of selection bias of younger Asian patients with less severe clinical disease. Our results should be verified in larger and more diverse groups of patients. Finally, the relationship of viral loads captured on FMS to replication-competent virus is uncertain and will be a key area for further research.
In conclusion, we present a novel, clinically compatible tool for the detection and quantification of exhaled SARS-CoV-2 in hospitalised patients with COVID-19. We detected potentially significant relationships between exhaled viral load, clinical presentation and outcome. Our findings provide a strong incentive to investigate this approach further, to understand the pattern and quantity of exhaled virus both in hospitalised and community cases, and to explore the potential of FMS to identify those who are most infectious.

Therefore, it is necessary to increase the added value of waste face masks through safe recycling and utilization. Face mask is mainly made of polypropylene non-woven fabric, which has a lot of fiber structure and porosity [5]. Therefore, the high-temperature carbonization of face masks can not only inactivate viruses but also be used to prepare porous carbon materials.
Carbonizing waste polymer materials is one of the most promising recycling methods [6]. Carbon is the main component of polymer materials, so carbon sources with added value, such as amorphous carbon (activated carbon, carbon fiber, carbon spheres), graphite, and carbon nanomaterials, can be produced by carbonization [7, 8]. Carbonized products are controllable and energy-saving emission friendly, and do not require complex pretreatment. Many polymer materials have been proven able to produce precursors of carbon materials through carbonization [9]. For example, polyolefins, a plastic waste, can be converted into carbon nanotubes under catalyzed conditions [10, 11], and polystyrene waste can be modified by carbonization for supercapacitor electrode materials. LDPE (low-density polyethylene) and HDPE (high-density polyethylene) can be carbonized to form carbon materials for lithium battery electrode materials after sulfonation [12]. Although carbonization based on polypropylene has been studied, the application of preparing electrode materials for supercapacitors is rarely looked into. Carbon materials are usually prepared with MgO as a template and add catalyst or through chlorination sulfonation modification [13–15]. However, these methods bring limited carbon production and have no potential application prospects.
In response to common problems like low carbon yield, low porosity, and low specific capacitance of carbon electrode in carbonization process, this study makes full use of the porous characteristics of polypropylene non-woven mask to prepare high-performance porous carbon materials. Sulfonated waste masks were prepared with solvothermal method in the autoclave, then mixed evenly with activator. Porous carbon materials were obtained after a one-step heat treatment. On the one hand, sulfonation can not only improve the carbon yield but also realize heteroatom doping to improve the physical and chemical properties of porous carbon materials. On the other hand, the pore-forming effect of activator can improve the porosity and enhance the capacitive performance of electrode materials.

When asked whether he will be able to play again at St Andrews, which happens to be one of his favorite venues, he replied:
"As far as retiring, no. But this may be my last chance I"ll be playing at St Andrews with a chance to compete at the highest level. If it comes around in five, six, seven or eight years" time, whatever the time frame is, who knows if I"ll be playing the game of golf at this level? I appreciate every single moment, especially coming back here."

“But that’s no longer my future. I’m not going to be able to do that again. So this is what my future is, a very limited schedule. I’m going to have to somehow figure out a way to practice at home efficiently and come to these events maybe just a little bit earlier and get more looks and try to get a better feel for it, and really trust what I’m doing because I hadn’t proven it to myself in any other tournaments prior to this." Despite his reduced calendar, he still plans on playing and has no plans of retiring. His comeback from the accident in 2021 was astonishing, but there is only so far he may go. He has vividly struggled with his right leg in all of the tournaments he has played post his accident. However, he is proud to be able to play in these tournaments after a career-threatening accident when it looked like he would never have the opportunity to play again. Last week, the embattled World of Warcraft and Call of Duty maker announced it would no longer require office-based staff to have had Covid jabs, while encouraging a gradual return to in-person working over the coming months. The decision, announced in an email by Activision Blizzard exec Brian Bulatao, was immediately criticised by staff who said it risked employees who may be immunocompromised or who lived with others who are, and was poorly timed as the US faces up to Covid"s current Omicron BA2 subvariant. Staff criticism sparked a slight climbdown from Activision Blizzard, in a further email from Bulatao. This delegated the decision on dropping vaccine mandates to individual studios, which allowed Blizzard to say it would keep the requirement in place for "at least the next few months". But this wasn"t enough to stop staff walkouts yesterday, with around 80 (via Axios) to 115 (via Polygon) staff apparently taking
part.
